How to Relocate Analog/INS-Net Services (Telephone Services)
A guide to moving your telephone subscription to a new location.

| Address | Please inform us of your address if it differs from the place where the telephone line is installed. |
| Preferred installation date | Requested dates for disconnection and reconnection |
- If you are using Itemized Billing Statement, you will have to re-apply for the service at an NTT West branch in your new location.
