(Procedures are handled by mail.)
Changing the name of the subscriber
Download the appropriate form for changing the name on the telephone subscription rights, fill in the required items, and send back by mail to NTT West with the required documents.

- After filling in the required items, affix with the seal of the inheriting party if an individual or the company seal of the new subscriber if a company.

- When the subscriber name changes due to marriage or other circumstances, or in the case of a company because of a corporate name change or reorganization, promptly send the NTT West Request for Transfer/Name Change form with the required documents.
- Separate procedures are required if the telephone line is leased or if you are paying for an installment contract, telephone directory advertisement contract, etc.
- You may be contacted to confirm documents or service details, so be sure to fill in your contact telephone number and name.
