A guide to procedures for changing the name on the subscription rights of Analog and INS-Net telephone services.
(Procedures are handled by mail.)
Transferring subscription rights
Download the appropriate form for changing the name on the telephone subscription rights, fill in the required items, and send back by mail to NTT West with the required documents.

- - Billed with telephone charges.
(Billed for telephone number transferred. Note that the fee cannot be added to a mobile phone bill.) - - If subscription rights to be transferred are currently suspended (Type II [Riyo Kyushi]), the fee can be added to a bill for a different telephone number with the same owner in the NTT West area, or billed individually.
If billed individually, transfer will be processed after the transfer approval handling fee is paid. - - If request for approval to transfer is made for usage rights for IP communications network services (FLET’S ISDN, FLET'S ADSL, B FLET'S (names of Internet access services), or FLET'S Office (option)) or usage rights for connection-only services (DSL services provided by other operators), a transfer approval handling fee for each case must be paid in the same manner.

Notes
- Separate procedures are required if the telephone line is leased or if you are paying for an installment contract, telephone directory advertisement contract, etc.
- You may be contacted to confirm documents or service details, so be sure to fill in your contact telephone number and name.
- If the transfer is approved, the new subscriber will assume all rights and obligations (such as for unpaid charges, etc. [including the transfer approval handling fee and current charges being calculated]).
- INS-Net 64 Lite and Analog Lite Plan (telephone services) cannot be transferred.
- The Request for Approval for Transfer form becomes valid upon approval by NTT West.
