Your credit card company will bill you for telephone charges along with other credit usage.

| Automatic Bank Account Payment |
 |
| NTT West will transfer the total amount due from the designated
bank account on the due date. For automatic bank account payment, please fill
out an application form, which requires the seal or signature you used to open
your bank account, and take the application form to your banking institution,
post office. |

Until your application is processed, it
is necessary for you to pay the bill sent by mail in person. |
| |
The amount which appears on the bill of NTT
West consists of charges for telephone services, telegram services, telephone
directory advertising, NTT West products etc. The following may also be included
in the amount: lease payments, installment payments, DIAL Q2 information
charges, CAPTAIN information charges etc. |

If NTT West cannot transfer the amount of
the bill due to insufficient funds, a second attempt at transfer will be made
ten days later. (Exceptions may apply for some banking institutions.) |

| Paying in Person |

NTT West bills should be paid by the due date at a bank, credit
bank, credit union, cooperative bank of commerce and industry, cooperative bank
of agriculture and forestry, labor credit association, agricultural cooperative
association, post office or a convenience store displaying

| Delinquent Accounts |

NTT West may disconnect your telephone line or terminate your
contract if payment is not received.
However, failure to pay DIAL Q2 information charges will not result
in the disconnection of your telephone line or termination of your contract by
NTT West. |

NTT West may charge 14.5% p.a. interest
(approximately 0.04% per day) as a penalty for payment made after the due date. |
