The telephone number will not be changed. During the suspension period, basic
monthly charges will apply. |

The telephone number will be changed when the line is reconnected. During the
suspension period, basic monthly charges will not apply.
Not available for telephone lines under Analog Lite Plan or INS-Net 64 Lite.

| Telephone Bill Payment after Suspension |

| After you suspend your telephone service, one or two bills may
be issued for charges up until the time it is actually disconnected. Subscribers
using automatic bank account payment are requested to pay in person if the bank
account will be closed before the final payment can be made. Please use the NTT
West bill with an electronic bar code. |
