	Changing the Name on the Subscription Rights

Please call 0120-064337 when changing the name on your telephone subscription rights. Applications are received at the NTT East/West Subscription Center (Kanyuken Center) in charge of your subscription. Applications can also be made by post. Note: Requests for relocating, suspending telephone service etc. are accepted only from the subscriber of the line. If the name of the person making the request differs from the name of the subscriber, the request cannot be carried out. Application Procedures There are three ways to change the name of the telephone line subscriber. Please check which category you fall into and carry out the relevant application procedures. Transfer of Telephone Subscription Rights Both the current and new subscribers should complete and sign the form 'Request for Approval for Transfer of Telephone Subscription Rights' (Denwa Kanyuken Joto Shonin Seikyusho) and submit this along with the necessary documents.   Individual Company Current Subscriber • Certificate of Alien Registration1 with current address • Seal/signature (Certificate of Signature1,2 verifying the current subscriber's signature is required when the current subscriber cannot go to the NTT West service counter in person.) • Certificate of Registered Seal Impression2 (Inkanshomeisho) and the seal • Applicant's employee identification New Subscriber • Certificate of Alien Registration1 with current address • Seal/signature • Certified Copy of Commercial Registration2 (Tokiboto(sho)hon) • Seal of the company's representative • Applicant's employee identification Handling fee ¥800 (¥840 tax included) • Transfer of telephone subscription rights is subject to the approval of NTT West. • Please be aware that any unpaid telephone charges become the responsibility of the new subscriber. • Not available for telephone lines under Analog Lite Plan or INS-Net 64 Lite. Acquisition of Telephone Subscription Rights When the telephone subscriber's name is changed due to inheritance or a corporate merger etc., please submit the form 'Request for Telephone Subscription Rights Transfer/Telephone Subscriber Name Change' (Kanyu Shokei Kaisho Todokesho) along with the necessary documents as soon as possible. Inheritance Merger • Certificate of Alien Registration1 with current address • Official documents which verify the situation • Seal/signature • Certified Copy of Commercial Registration2 (Tokiboto(sho)hon) which verifies the company merger • Seal of the company's representative • Applicant's employee identification Change in Subscriber's Name When there is a change in an individual's or company name due to marriage, company reorganization etc., please submit the form 'Request for Telephone Subscription Rights Transfer/Telephone Subscriber Name Change' (Kanyu Shokei Kaisho Todokesho) along with the necessary documents as soon as possible.
